Coughing Pom

Hello all!
My Pom has been coughing for 4 months. Vets are rare and have to come from 6 villages away. The last vet said he just needed to be dewormed. It has been over 3 weeks since the deworming, but my poor dog is no better and keeps coughing repeatedly. He tries to bring sputum out when he coughs. What should I do?
