coughing, 10 days of azithromycin with no changes, lung ex ray shows something

My Pomeranian is a male age 4 to 5 yrs. He has been having this coughing spell for the past 2 weeks. The vet put him on azithromycin for (it’s been 10 days with no change) The lung ex ray shows something, she thinks could be bronchitis? Well please tell me some other worse case senerios, or other possibilities for his coughing since the antibiotics haven’t phased him.
